If you’re looking at this page, it’s likely that the surgery sent you a link to it because your child may need a referral for assessment for either ADHD or autism spectrum.
For the surgery to make the referral, there are two forms that need to be filled out and returned to us.
The first form, called SNAP-IV, is a rating scale that should be filled out by one of the child’s parents and also one of the child’s teachers.
The second form needs to be filled in by the school. Usually the SENCO (Special Educational Needs Officer) does this.
Please remember to send both forms back to the surgery. The easiest way to do this is to reply to the SMS message we sent you with the link to this page. If that doesn’t work, use PATCHs to send it to us. The PATCHs link is on the surgery home page.
Once a GP has reviewed the forms we will contact you. Thanks for helping us collect this information which will be used by the team we make the referral to.
