If you live within our catchment area and wish to register please enquire at reception. You will need to bring proof of your identity, address and details of your previous GP (if applicable). Your registration will not be processed without these documents.

Visitors to the UK may not be eligible for NHS services - please check with reception.

Children

If you have a child under six that you wish to register, please remember to bring their hand-held records (red book) or written confirmation of their immunisations as this will assist our health visitor.

Temporary Patients

Patients who are staying in the area for a short time and are entitled to NHS treatment may register as temporary patients. Please discuss this with the receptionists.

PreRegistration If you wish to register at the practice you will be asked to complete a registration form and you will need to bring proof of address* and proof of identity with you the first time you visit. You will also be asked to sign the registration form to confirm that the details are correct.
* NB Please note that mobile phone bills are not an acceptable form of proof of address.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
